Wstęp:
Pojęcie „psychosomatyki”, zostało wprowadzone do użycia w pierwszej połowie XIX wieku. Aktualnie obowiązująca klasyfikacja DSM-IV (Międzynarodowa klasyfikacja Amerykańskiego Towarzystwa Psychiatrycznego) traktuje zaburzenia psychosomatyczne jako skutek działania bodźców psychologicznych, które są czasowo powiązane z pierwszymi objawami lub zaostrzeniem choroby psychosomatycznej. Główną przyczyną choroby wrzodowej jest nieleczona infekcja Helicobacter pylori. Uważa się, że przyjmowanie niesteroidowych leków przeciwzapalnych może przyczynić się do postępu zmian chorobowych. Celem pracy była analiza udziału emocji, osobowości, stresu w patogenezie choroby wrzodowej żołądka i dwunastnicy. Mówiąc o patogenezie choroby wrzodowej i dwunastnicy nie można jednak pominąć wypływu czynników psychologicznych. Wprawdzie pojedyncza sytuacja stresowa nie spowoduje trwałych zmian organicznych, ale długotrwała ekspozycja na stresor może skutkować rozwojem choroby wrzodowej. Nawet 30% do 65% wrzodów żołądka i dwunastnicy jest spowodowanych stresem. Do choroby wrzodowej predysponuje typ „D” osobowości, charakteryzujący się zaburzonym funkcjonowaniem społecznym oraz trudnościami w wyrażaniu emocji. W związku z tym, iż choroba wrzodowa żołądka i dwunastnicy jest zaburzeniem o podłożu psychicznym, w terapii nie powinno zapominać się o pomocy psychologicznej. Pacjenci powinni uczęszczać na spotkania z psychoterapeutą w celu poznania i nauczenia się sposobów radzenia sobie ze stresem.


Introduction:
Introduction: The concept of ‘psychosomatics’ was introduced in the first half of the 20th century. According to the American Psychiatric Association (DMS-IV classification), psychosomatic disorders are the effects of psychological factors precisely connected with the beginning or exacerbation of an illness. The main causes of chronic peptic ulcer disease are Helicobacter pylori infection and non-steroid antiinflamatory drugs.

Objective:
Analysis of the role of emotional factors, stress and type of personality in the pathogenesis of chronic peptic ulcer disease. The major role of psychological factors should not be ignored. One stressful situation cannot cause a peptic ulcer. Through the years, the human organism has gained many defence mechanisms (gastric prostaglandins, regular blood flow in gastrointestinal vessels) protecting against stress. Nowadays, damage to this barrier is more common and the amount of harm is still increasing. About 30-60% of gastric and duodenal ulcers are said to be the result of stress. Chronic peptic ulcer disease is more common among people with type D personality, characterized by a social dysfunction and tendencies for hiding emotions. This is the reason why psychotherapy should be performed as part of the treatment. Other mental disturbances causing progression of the ulcer are anxiety and depression. Due to the co-existence of a somatic and a mental background to the illness, psychological support should be provided. During meetings with the psychotherapist, the patients can learn how to deal with the stress of everyday life.
